Heart Failure Advanced Certification

Grand Lake Health System has received the Heart Failure Advanced Certification (HFAC) from DNV. The certification affirms an organization’s excellence in heart failure diagnosis and treatment inclusive of initial diagnostic services, and therapies related to heart failure – as well as clear metrics to evaluate program outcomes. We are the first hospital in Ohio to receive this certification and 1 of 8 in the nation.

Acute Stroke Ready Center

Grand Lake Health System is a certified Acute Stroke Ready center for demonstrating excellence and complying with standards of care for the initial treatment of stroke patients. If you are experiencing signs of a stroke, including numbness, confusion, severe headaches, one-sided arm or leg weakness, slurred speech or have trouble walking or seeing, call 911 immediately.

ISO 9001 Quality Management Certification

The ISO 9001 standard provides specific requirements for a quality management system that will enhance your ability to consistently deliver products and services that meet customer, as well as statutory and regulatory, demands. ISO 9001 set out the criterial for a quality management system which focuses on the effectiveness of the quality processes, helping you work more efficiently and reduce product failure.

2025 Healthy Worksite Gold Level Employer

Grand Lake Health System was recognized by The Healthy Business Council of Ohio (HBCO) as a Gold level award winning for healthy worksite practices. The Healthy Worksite Award recognizes Ohio employers who demonstrate a commitment to employee wellness through comprehensive worksite health promotion and wellness programs. Applicants are scored on the extent to which their wellness programs facilitate and encourage employee health, enhance productivity, and ensure a healthy work environment.

ISO 9001:2015 Certification

ISO 9001:2015 certification demonstrates that an organization has a robust Quality Management System (QMS) in place to consistently deliver products and services that meet customer and regulatory requirements. This internationally recognized standard sets specific requirements for a QMS, helping organizations improve their processes, reduce waste, and enhance customer satisfaction.

American Association of Cardiovascular and Pulmonary Rehabilitation Certification

The American Association of Cardiovascular and Pulmonary Rehabilitation (AACVPR) certification is a recognition of a hospital’s pulmonary rehabilitation center’s commitment to providing patients a high standard of care and improving their quality of life. A successful pulmonary rehabilitation program requires healthcare providers to undergo training, which involves completing a series of 12 modules to become an effective member of a pulmonary rehabilitation team.

National Certified Gold Safe Sleep Champion

Grand Lake Health System achieved the highest certification level of Gold Safe Champion through our commitment to community leadership for best practices and education on infant safe sleep through the development of a hospital policy, staff training, parent education, implementing a wearable blanket program, becoming a Cribs for Kids partner, and a pledge to participate in ongoing audits and community outreach programs.

Medicare 5 Star Rated Hospital

This award recognizes hospitals that achieve 5 stars in quality for the care they provide. It recognizes those hospitals that achieve high measures in quality, such as: mortality, safety of care, readmissions, patient experience, and timeliness and effectiveness of care. Center of Distinction Award by Healogics®

Grand Lake Wound Care Center has been awarded the prestigious Center of Distinction award by Healogics®, the nation’s largest provider of advanced wound care services. The Wound Care team achieved outstanding clinical outcomes for twelve consecutive months, including a patient satisfaction rate higher than 92 percent, wound adjusted comprehensive healing rate greater than or equal to 77% and an outlier rate less than or equal to 16%.

Robert A. Warriner III, M.D., Clinical Excellence Award by Healogics®

Grand Lake Wound Care Center has been awarded the prestigious Robert A. Warriner III, M.D., Clinical Excellence Award by Healogics, the nation's largest provider of advanced wound care services. Grand Lake Wound Care Center has scored in the top 10 percent of eligible Healogics® Wound Care Centers® on the Clinical Excellence measure. To be awarded the Clinical Excellence Award, a Center must have a Wound Adjusted Comprehensive Healing Rate of 83% or higher from December 2023 to November 2024.

Miami & Erie Medical Center construction completed
