Grand Lake Home Health has been providing compassionate and comprehensive care since 1987, helping patients of all ages recover and regain independence in the comfort of their own homes across Auglaize, Mercer, Allen and Shelby Counties. Our team, including registered nurses, home health aides, physical, occupational, and speech therapists, and medical social workers, works together under a doctor’s personalized care plan. Grand Lake Home Health is accredited and Medicare certified. Our business office conveniently files all insurance billing, including Medicare.
We provide a five-star range of services to fit
any medical situation – right in your home:
- Cancer Care: non-hospice or pre-hospice emotional, social and physical support and treatment.
- Chronic Illnesses: blood draws, medication management and education about the disease or illness.
- Emergency Care: full range of services for situations not requiring hospital transport.
- Family Care: well baby assessment, new mom education, teenage illnesses and injuries.
- Personal Physical Support: meal preparation, diet consultation, personal hygiene and exercise.
- Stroke Victims: rehabilitative services including physical, occupational or speech therapies.
- Surgery Recovery: wound or ostomy care, IV therapy, IM and SQ injections and catheter management.
