Benefits of Diabetes Self-Management Education
Our diabetes self-management education program is focused on providing education to empower clients on how to effectively manage diabetes, which can help improve clinical outcomes and quality of life. Our goal is for each individual to understand the diabetes disease process and the options available to manage it. Then, individuals can make informed decisions about what may work best for them to manage their diabetes, in collaboration with their physician.
What is the Diabetes Education Program?
Education sessions to meet your individual needs to promote a healthier lifestyle and improve management of diabetes. Clients are scheduled for an initial consult with the nurse diabetes educator/dietician for an assessment of individual needs and then education is provided based on identified needs, along with setting realistic goals. Additional follow-up consults vary depending on each individual’s needs but are important to help evaluate progress and provide reinforcement and support. We want to ensure that when you complete our program, you feel confident in your ability to manage your diabetes.
What Topics are Covered by the Program?
- Understanding what diabetes is and options for managing it
- Healthy eating, including carb counting
- Physical activity
- Testing blood glucose and using results
- Understanding medications, including insulin
- Ways to reducing risk for developing diabetes complications
- Realistic goal setting

Your physician can schedule your appointment through our Scheduling Department (419-394-3387 ext. 3550) or through Krissy Mullenhour, RN, BSN, CDE (419-394-3387 ext. 2147)
Physician referral is required for all consults. Our program can contact your physician’s office, if needed.
